Lighthouse Montessori School

Required for all students. For new students: all enrollment paperwork will be accepted only after the entire application procedure has been followed and you have been notified of your child's acceptance. Filled out enrollment forms plus registration fee will hold your child's place. If you have more than one child, you only need to fill this form out once. Please put each child's name on this form. Please check to make sure that you have signed each signature line. If you would like to add more than 5 names to your emergency contacts/child pick up list, please request an Emergency contact/child pick up extension form from our office.
